About The Position
Help patients overcome barriers to care, connect to community resources, and reach their health goals—all while making a real impact across Hawaiʻi Island. You’ll work alongside a supportive interdisciplinary team in a value‑based care model designed to improve health outcomes, strengthen patient engagement, and reduce avoidable ER visits and hospitalizations. Requirements
- High School Diploma or GED
- At least 2 years of experience in health or social services
- Strong knowledge of community resources
- Valid Hawaiʻi driver’s license, insurance, and reliable transportation
Nice To Haves
- Bachelor’s degree in a human services‑related field
