About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on reimagining care for children with special healthcare needs. They provide 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to overcome barriers to quality care. This role is part of the Transitions of Care Team and involves providing telephonic and in-person patient support for post-hospital care. The primary location is remote, with a preference for candidates living in TX, FL, MO, NC, GA, TN, NY, and/or GA.

Requirements

  • Bachelor’s degree required.
  • 3+ years of experience in a healthcare setting (patient-facing hospital or facility role).
  • Passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve.

Nice To Haves

  • Bilingual (Spanish) strong preferred.
  • National CHW certification preferred.
  • CPR certification preferred.
  • ER setting preferred.

Responsibilities

  • Work as part of an interdisciplinary, collaborative team to ensure patient needs are met.
  • Outreach to patients discharged from emergency rooms to ensure continuity of care and assist with identifying and meeting needs related to social determinants of health.
  • Deliver resources to families within your geographic area.
  • Coordinate with facility discharge planners to ensure no barriers to patients obtaining after-hospital resources and care.
  • Attend daily huddles with the team to communicate patient needs and progress.
  • Liaise with hospitals and other health systems to make staff aware of Imagine Pediatrics program and support.
  • Support patients with in-person/community needs related to care coordination/discharge support, such as pharmacy fills, PCP coordination, SDOH needs, and home visits.
  • Perform other duties as assigned.
