About The Position

The Patient Navigator plays a critical role in supporting individuals at risk for HIV by facilitating access to pre-exposure prophylaxis (PrEP) services and resources. This position focuses on educating clients about PrEP, assessing their eligibility, and guiding them through the healthcare system to initiate and maintain PrEP treatment. The Patient Navigator collaborates closely with healthcare providers, community organizations, and clients to ensure adherence and address barriers such as stigma, insurance, and transportation. By providing personalized support and follow-up, the Patient Navigator helps reduce new HIV infections and promotes overall sexual health within diverse communities. Ultimately, this role contributes to public health goals by increasing PrEP uptake and retention among populations at elevated risk for HIV transmission.

Requirements

  • High school diploma or equivalent
  • Experience working in HIV prevention, sexual health, or community health outreach.
  • Strong interpersonal and communication skills with the ability to engage diverse populations.
  • Basic knowledge of PrEP, HIV transmission, and related healthcare systems.
  • Ability to maintain confidentiality and handle sensitive information with professionalism.

Nice To Haves

  • Associate or bachelor’s in public health, social work, or related field preferred.
  • Bachelor’s degree in public health, social work, nursing, or a related discipline.
  • Experience working with LGBTQ+ communities and other populations disproportionately affected by HIV.
  • Familiarity with insurance processes, medication assistance programs, and healthcare navigation.
  • Bilingual abilities, particularly in Spanish or other languages commonly spoken in the community.
  • Certification or training in HIV counseling, case management, or patient navigation.

Responsibilities

  • Conduct outreach and education to raise awareness about PrEP among at-risk populations.
  • Screen and assess clients for PrEP eligibility based on medical history and risk factors.
  • Provide one-on-one counseling to clients regarding PrEP benefits, side effects, and adherence strategies.
  • Assist clients in navigating healthcare systems, including scheduling appointments, insurance enrollment, and medication access.
  • Maintain detailed records of client interactions, follow-ups, and outcomes to monitor program effectiveness.
  • Collaborate with healthcare providers, social service agencies, and community organizations to coordinate comprehensive care.
  • Identify and address barriers to PrEP initiation and adherence, including stigma, transportation, and financial challenges.
  • Participate in community events and training sessions to stay informed about the latest PrEP guidelines and best practices.
