Short Description Bridge the gap between clinician and patient, by providing individual support and assistance in navigating the health care system to provide continuity of care. Working with current RN Navigator disease team to provide non-clinical support regarding scheduling, pre cert process, follow up calls and record retrieval for timely appointments Call new patients within 24 hours of visit to CCI to check in regarding follow up appointments and needs.

Requirements

  • Experience working in a healthcare office setting, or community outreach and education required.
  • Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population
  • Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
  • Ability to take direction, follows through with tasks, and work independently.
  • Flexibility, optimism, and a cooperative working style.
  • High School Diploma or Equivalent; clinical training (i.e. MA, CNA, LPN) a plus.
  • Competent in basic computer skills; knowledge of using an electronic health records system preferred.

Nice To Haves

  • Fluency in Spanish preferred.
