About The Position

At NewYork-Presbyterian Hospital, we are committed to delivering excellence in clinical care, patient safety, education, clinical research, and science. As part of our mission, the Patient Navigator Program empowers and educates patients on accessing and utilizing healthcare services. Our goal is to support patients and caregivers in creating effective care plans and to connect them with the right health and social services to optimize their well-being. The Patient Navigator serves as the primary care coordinator for patients, working closely with key clinical staff to ensure comprehensive care and treatment. The Patient Navigator Program is based in the Emergency Department, where it serves both pediatric and adult populations, ensuring that each patient’s journey through healthcare is as smooth and informed as possible. You’ll play a crucial role in enhancing patient experience and access to care. This is a full-time position based out of our NYP/Queens site. The shift is 3:00pm-11:00pm with alternating weekends required.

Requirements

  • High School Diploma or GED
  • Previous experience in healthcare
  • Understanding of medical office practices and procedures
  • Strong organizational, multi-tasking, and time management skills
  • Flexibility, persistence, and patience in a dynamic environment
  • Excellent oral, written communication, and interpersonal skills
  • Proficiency in Microsoft Word and Excel

Nice To Haves

  • Bachelor's degree
  • 2-3 years of experience in primary or community health
  • Familiarity with hospital operations hospital and community resources
  • Fluency in Mandarin, Cantonese, or Spanish

Responsibilities

  • Scheduling and managing patient appointments
  • Providing logistical support
  • Facilitating communication between patients and the medical team
  • Addressing patient inquiries
  • Directing them to the appropriate staff
  • Assisting with the collection of medical records, insurance details, and financial information
  • Engaging with patients in person, over the phone, and via email to ensure seamless coordination of care
