About The Position

Windrose Health Network is seeking a Patient Navigator (Interpreter) who is bi-lingual in Burmese and Hakha Chin to join their team. The Patient Navigator facilitates patients' utilization of WHN's services and healthcare services in the community. Responsibilities include assisting medical staff with care delivery by scheduling appointments, providing interpretation services, assisting with forms, and ensuring patients understand care recommendations. Externally, the PN helps patients access referred services by arranging appointments, providing directions/maps in the patient's native language, and arranging transportation when necessary.

Requirements

  • At least one (1) year of post-high school education, preferably in a healthcare-related discipline such as a Medical Assistant (MA), Community Health Worker (CHW) or social services.
  • At least two (2) years of experience in a healthcare or social services setting, preferably an outpatient primary care clinic.
  • Working knowledge of processing software package (preferably MicroSoft Word).
  • Working knowledge of one spreadsheet development software package (preferably MicroSoft Excel).
  • Fluent in Burmese, Hakha Chin, and English language.
  • Knowledge of the delivery of ambulatory medical care.
  • Written and spoken language proficiency, in a language other than English, sufficient enough to serve as a medical interpreter for Health Center patients.
  • Knowledge of the daily operations of a primary care, outpatient medical clinic, especially when it comes to making referrals for specialty medical care.
  • Customer service and telephone skills.
  • The ability to plan and manage the utilization of resources.
  • Knowledge of the rules and regulations related to HIPAA and Corporate Compliance.

Nice To Haves

  • Working knowledge electronic medical record (EMR).
  • Experience with interpreting/translating.
  • Certification as a Medical Interpreter.
  • Knowledge of and skill with the WHN's electronic medical record and practice management system.

Responsibilities

  • Facilitates patients’ utilization of WHN’s services.
  • Helps expedite WHN patients’ use of healthcare services in the community.
  • Assists the medical staff with care delivery by scheduling medical appointments.
  • Provides interpretation services.
  • Assists with the completion of forms.
  • Ensures patients understand care recommendations from their medical provider.
  • Helps ensure patients can access the services to which they have been referred by arranging appointments.
  • Provides directions and/or maps in the patient’s native language.
  • Arranges for transportation to and from medical appointments when necessary.
