The Patient Navigator guides women’s health patients through the health care system to help reduce late-stage breast and cervical cancer diagnoses. This role supports access to care, builds relationships with service providers, and tracks interventions and outcomes. The Patient Navigator promotes the mission, vision, and values of FoundCare. They facilitate internal and outgoing referrals for mammogram screenings, work with the Care Team to communicate with patients about abnormal findings, and connect patients to community and social support services. Additionally, the role involves providing women’s health education, building relationships with other patient navigators, assisting patients with payment for care, arranging transportation, tracking interventions and outcomes, identifying the target population, establishing community relationships, and using culturally and linguistically appropriate strategies. The role requires working flexible evening and weekend hours as needed and contributing to program goals. The Patient Navigator will also maintain a log of outgoing referrals, ensure authorizations are acquired, and update records for reporting requirements.
