About The Position

The Patient Navigator is an ideal opportunity for those seeking to be an integral part of a fertility patient’s journey toward becoming a parent. The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care. We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Michigan in Bloomfield Hills, MI. This is a full-time, on-site position working from Monday-Friday from 8:00AM - 4:30 PM schedule with some holidays required.

Requirements

  • Highschool Diploma required.
  • Minimum of 2 years of experience in customer service, patient navigation, case management, or a related healthcare role.
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.
  • Proficiency in using electronic medical records (EMR) and other healthcare software.

Nice To Haves

  • Bachelor’s degree (preferred)
  • Bilingual skills are a plus.

Responsibilities

  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
  • Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and help resolve any issues or barriers to care.
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
  • Other duties and project as assigned.
