About The Position

Central Maine Healthcare (CMH), a member of Prime Healthcare Foundation, is an integrated healthcare delivery system serving approximately 400,000 residents across central, western, and midcoast Maine through a network of more than 40 primary and specialty care locations. Requirements

  • Bachelor's degree or four (4) years of applicable cumulative job specific experience required.
  • Current BLS certificate upon hire and maintain current.

Responsibilities

  • Coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community.
  • Coordinating the care in a timely and efficient manner, identifying and eliminating barriers to care.
  • Acting as a consistent care coordinator throughout the continuum of care.
  • Assessing the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care.
  • Community education and screening.
  • Monitoring community outreach and promoting survivorship.
