Responsibilities
- Administer validated rating scales.
- Initiate behavioral health care planning concerning behavioral or psychiatric health problems.
- Revise or modify care plans for patients who are not progressing or whose status changes.
- Provide brief psychosocial interventions as needed.
- Collaborate on an ongoing basis with the billing practitioner.
- Maintain the registry and tracking sheets.
- Consult with the psychiatric consultant.
- Maintain a continuous relationship with patients.
- Foster collaborative, integrated relationships with the rest of the care team.
- Conduct interdisciplinary care plan meetings to review patient beneficiaries.
- Review patient charts and communicate irregularities to the Clinical department and providers.
