About The Position

The Patient Flow Assistant supports the coordination of patient transfers into UHS Hospitals, ensuring timely placement at the appropriate level of care. This role collaborates closely with Physicians, Nursing Supervisors, and referring facilities to facilitate physician-to-physician communication, monitor transfer progress, and maintain efficient patient flow across the organization. Serving as a key liaison, the Patient Flow Assistant provides high-quality customer service while coordinating communication among multiple stakeholders via phone, electronic systems, and in-person interactions. This position is responsible for completing and maintaining Transfer Center documentation, entering data for reporting purposes, and adhering to established policies and procedures to support safe, seamless patient transitions. Requirements

  • High School Diploma
  • 1 year experience working in a healthcare related field.

Responsibilities

  • Assists the Bed Census Management Nurse and Nursing Supervisor with coordinating patient transfers from outside facilities, ensuring appropriate placement based on level of care.
  • Facilitates physician-to-physician communication throughout the patient transfer process.
  • Collaborates with interdisciplinary healthcare team members to coordinate efficient and timely patient transfers.
  • Monitors transfer timelines, maintaining awareness of delays and providing timely status updates to all stakeholders.
  • Serves as a liaison between UHS Hospitals and referring/sending facilities to ensure clear and consistent communication.
  • Provides high-quality customer service via phone, electronic communication, and in-person interactions.
  • Partners with Physicians, Nursing Supervisors, and external facilities to accurately complete Transfer Center documentation.
  • Collects, enters, and maintains Transfer Center data to support reporting and operational tracking.
  • Adheres to established organizational policies, procedures, and regulatory standards.
  • Performs other duties as assigned in support of departmental and organizational needs.
