About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Patient Experience Coordinator to join the Nursing Administration team. This full-time position, based in Clarksville, operates within a community dedicated to advancing health and wellness through patient care, education, and research. The role involves applying the nursing process to manage complex patients and programs, providing consultative services, and collaborating with the healthcare team. The coordinator will address complex learning needs, conduct effective patient and staff education, and demonstrate effective communication to coordinate patient and family-centered care. Responsibilities include facilitating appropriate follow-up care, advocating for patients and families, ensuring evidence-based care, and monitoring outcomes. The position also requires continuous professional growth in line with Board of Nursing standards.

Requirements

  • Relevant Work Experience
  • 3 years experience
  • Bachelor's degree

Responsibilities

  • Collaborates with interdisciplinary team, attending physicians, families and other professionals to formulate and initiate specialty norms and maintain collaborative practice agreements.
  • Utilizes advanced clinical knowledge to provide or guide safe and effective continuum of care.
  • Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care and program needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient and program progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Additional duties may be assigned.
