About The Position

At H2 Health, care and coordination are essential to patient success. The Patient Coordinator bridges patient care with administrative excellence and serves as the first point of contact for patients, providing outstanding customer service and helping ensure smooth clinic operations. The position is based in an outpatient clinic at 4820 W Deer Lake Dr D-9, Jacksonville, FL 32246. The role coordinates appointments, verifies insurance, handles referrals, maintains accurate and confidential patient records, supports front desk operations, answers calls and inquiries, and works with clinicians and administrative staff to improve the patient experience. The coordinator also helps maintain a welcoming and organized clinic environment.

Requirements

High school diploma or equivalent.
At least 1 year of experience in a healthcare or medical office setting.
Knowledge of medical terminology and insurance processes.
Exceptional interpersonal and communication skills.
Familiarity with Electronic Medical Records (EMR) systems and basic computer skills.
Ability to handle sensitive information confidentially.
Proficiency with office software and electronic health records (EHR) systems.

Nice To Haves

Additional education in healthcare administration.
