About The Position

Youngs is a mission-driven home care and adult daycare organization specializing in one-on-one personal care, companionship, and culturally responsive services. We design personalized care plans that support clients transitioning from hospital to home, as well as seniors seeking meaningful engagement through adult daycare. The Patient Coordinator is a key member of the Adult Daycare office team and supports both Adult Daycare and Home Care operations. Reporting to the Adult Daycare Director, this role is central to patient engagement, retention, and care coordination. This position is especially impactful within the Korean community, where language fluency, cultural understanding, and trusted relationships are essential to delivering exceptional care.

Requirements

  • Bilingual in Korean and English (required)
  • High school diploma or equivalent
  • Proficient with Microsoft 365 and basic computer systems
  • Ability to work full-time hours
  • Ability to lift up to 20 pounds and perform light physical activity throughout the day
  • Ability to perform essential job functions with or without reasonable accommodation

Responsibilities

  • Serve as the primary point of contact for patient concerns and feedback, resolving issues proactively to enhance satisfaction and reduce attrition
  • Track, analyze, and report client retention, attendance, and billable hours KPIs
  • Monitor daily attendance and identify trends or irregularities
  • Contact-and when appropriate, visit-patients who have stopped attending to encourage re-engagement
  • Maintain accurate master attendance logs and meal records
  • Coordinate and assist with snack and lunch preparation
  • Collect, organize, and track required documentation including TB tests and physical exam records
  • Manage and distribute patient gifts and recognition for attendance, birthdays, and activities
  • Leverage community relationships-particularly within the Korean community-to support patient care and staffing needs
  • Coordinate with Personal Care Assistants (PCAs) to assist with emergency coverage
  • Step in to temporarily fill shifts when necessary to ensure uninterrupted patient care
  • Support the Home Care Coordinator with recruitment, placement, and staffing fulfillment
  • Track and report on home care KPIs including vacant shifts
  • Uphold company policies, regulatory requirements, and iCARE values
  • Perform additional duties as assigned to support operational excellence
