About The Position

Town Square Health is reimagining what healthcare can be. We’re building a first-of-its-kind value-based care model with a singular mission: to set the gold standard for how Americans experience healthcare. Our primary care locations serve Medicare-eligible patients, and we’re growing fast, with expansion into multiple markets on the horizon. If you’re bold, collaborative, and driven to make a real difference in people’s lives, we’d love to have you on our team! This is not a traditional medical assistant or LPN role. As a TSH Navigator, you’ll be a vital member of our care team; a trusted partner and advocate for a dedicated panel of patients throughout their healthcare journey. You’ll go beyond the standard duties of a medical assistant to build real relationships, learn each patient’s individual needs, and ensure their care is well-coordinated and genuinely personal. You will be the primary point of contact for your patients–whether in the clinic, or between visits. Patients will count on you to answer questions, coordinate appointments, address concerns, and connect them with the right resources at the right time. You’ll have the opportunity to: Serve as a consistent, trusted guide for a panel of Medicare patients Bridge clinical care and community resources to remove barriers for patients Help define what seamless, proactive, patient-centered care looks like as we scale Be part of building something new–a care model designed from the ground up to do better You’ll be at the intersection of clinical excellence and patient advocacy, blending hands-on medical assistant duties with proactive care coordination.
