Requirements
- Caring for medically complex older adults during their most vulnerable health transitions.
- Improving patient outcomes, reducing hospital readmissions, and improving quality of life.
- Working closely with facility staff, families, and interdisciplinary teams to ensure seamless, compassionate, and coordinated care.
- Practicing meaningful medicine while making a tangible difference in patients’ lives.
- Leading, innovating, and contributing to the growth of geriatric care in our community.
Responsibilities
