About The Position

You’re passionate about guiding patients through every step of their care. As a Patient Care Navigator at Ohioans Home Healthcare, your clinical expertise, strong assessment skills, and ability to coordinate care are key assets in supporting exceptional patient outcomes. In this role, you’ll help ensure patients receive timely, high-quality care while serving as an important connection between our patients, physicians, and clinical team. You’ll apply your clinical judgment and nursing knowledge to support patient care, collaborate with healthcare providers, and provide guidance to our field staff when patient needs arise. We’ll rely on you to be a knowledgeable resource, respond thoughtfully to changing patient needs, and help ensure our team has the information and support needed to provide excellent care. Your ability to balance clinical priorities with our patient-first culture will play an important role in maintaining the quality experience we’re known for.

Requirements

  • Current, active licensure appropriate to duties performed: Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing.
  • Licensure must be valid in all states serviced by the organization where applicable.
  • Must be agreeable to get additional state licensure where needed within 90 days of employment.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Demonstrates the ability to work independently as well as within a team environment.
  • Demonstrates good communication, customer service, negotiation, and public relations skills.
  • Demonstrates excellent observation, verbal, and written communication skills.
  • Demonstrates the capability to accurately manage detailed information.
  • Able to deal tactfully with co-workers, patients, family members, referral sources, payers, and the community.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Nice To Haves

  • Minimum of one (1) year of experience in home health, wound care, medical supplies/equipment, and insurance processes preferred.
