About The Position

Aegis Home Health & Hospice is seeking a motivated and relationship-focused Patient Care Navigator/Healthcare Marketer to join our growing team. This position plays a vital role in connecting patients, families, and referral partners with compassionate home health and hospice services throughout the community. Requirements

At least two years of healthcare marketing, referral development, or relationship-building experience.
Excellent communication and organizational skills.
Self-motivated and goal-driven.
A valid driver's license.
Reliable transportation.
Auto insurance.
CPR certification.
Willingness to obtain a negative TB test.

Nice To Haves

Previous experience in hospice, palliative care, or home health is strongly preferred.

Responsibilities

Build and maintain referral relationships within the healthcare community.
Educate patients, families, and providers about Aegis Home Health & Hospice services.
Collaborate closely with clinical teams to support smooth patient transitions into care.
Serve as a trusted liaison between referral sources, patients, and caregivers.
Engage in community outreach and networking.
Help expand awareness of Aegis services throughout the Omaha and Lincoln markets.
