Aegis Home Health & Hospice is seeking a motivated and relationship-focused Patient Care Navigator/Healthcare Marketer to join our growing team. This position plays a vital role in connecting patients, families, and referral partners with compassionate home health and hospice services throughout the community. Requirements

  • At least two years of healthcare marketing, referral development, or relationship-building experience.
  • Excellent communication and organizational skills.
  • Self-motivated and goal-driven.
  • A valid driver's license.
  • Reliable transportation.
  • Auto insurance.
  • CPR certification.
  • Willingness to obtain a negative TB test.

Nice To Haves

  • Previous experience in hospice, palliative care, or home health is strongly preferred.

Responsibilities

  • Build and maintain referral relationships within the healthcare community.
  • Educate patients, families, and providers about Aegis Home Health & Hospice services.
  • Collaborate closely with clinical teams to support smooth patient transitions into care.
  • Serve as a trusted liaison between referral sources, patients, and caregivers.
  • Engage in community outreach and networking.
  • Help expand awareness of Aegis services throughout the Omaha and Lincoln markets.
