About The Position

The Patient Care Manager (PCM) position completes the client case history (intakes) for all sites per assigned location, schedules new client evaluations, manages conversion meetings, monitors referrals and admits daily, ensures patients complete initial visits, as well as maintains the scheduling information about therapists on staff, such as availability and areas of expertise. Team Responsibility: Treat fellow administrative team members, clients and therapists with respect and politeness. To come to work on time and follow established dress code policies. Operational Responsibility: 1. Procure patients for admission to Cole Pediatric Therapy. Obtain prospective client intake information (case history) during client consultation, and schedule the initial evaluation/s. Verify commercial insurance and enter information accurately into electronic medical records software. 2. Verify and communicate insurance benefits with prospective clients and document information accurately. 3. Case Management Coordination to include the following: a. Coordination with physicians for attainment of required documentation (i.e. diagnosis paperwork, testing, etc.) b. Coordination with Business Development Managers as required. c. Attendance of evaluations and rescheduling when applicable. d. Confirmation calls for initial evaluations. e. Educating parents on the importance of therapy and adhering to the plan of care. f. Coordination of cold cases. 4. Perform consultations for walk- in referrals. 5. Must meet minimum sales productivity standards as set forth by the Company. 6. Represent Cole Health in a professional, ethical and respectable manner. 7. Develop and maintain customer Profiles on the Cole Health share drive. 8. Cooperate fully with colleagues to ensure referrals & admissions are coordinated in a timely and efficient manner. 9. Internal business development 10. Procure physician satisfaction reports and summarize findings. 11. Manage other projects as assigned through completion. 12. Receive and review Client Intake from Billing Company prior to evaluation appointment to ensure the client has been apprised of insurance benefits. 13. Upon receipt of new referrals from physicians via fax, call the client to perform a telephone consultation. 14. Follow up with prospective clients who have been unreachable by telephone with an e-mail or letter as per Standard Work. 15. Attend meetings to discuss, and provide written report about, the progress of prospective clients and clients scheduling, as well as scheduling issues or concerns. Maintain daily metric reports on display as described in Standard Work. 16. Throughout the day assist with checking the e-fax website for referrals, signed reports and any other communication from physicians. 17. Follow standard work established by the Cole Pediatric Therapy. Alert your supervisor if the standard work needs to be reviewed. Other Responsibilities: Carry out any other projects as assigned by the Director of Revenue Cycle and Systems Management and Division Director.

Requirements

  • Bilingual English/Spanish required
  • 1-2 years experience in patient administrative care required
  • Microsoft Office Experience required
  • Must be a team player.
  • Must be dependable, friendly, outgoing and polite.
  • Bachelor’s Degree or equivalent job experience (3-5 years).
