As the Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.

Requirements

  • Current Physical Therapist or Occupational Therapist licensure in state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Nice To Haves

  • Current CPR certification or ability to complete within 90 days of hire

Responsibilities

  • Oversees patient admissions and care coordination, including referral intake, clinician assignment, Start of Care (SOC) planning, insurance authorization, and verification of home health eligibility and medical necessity
  • Manages and ensures the development, review, and ongoing updates of individualized patient plans of care in accordance with physician orders, regulatory requirements, and best practices
  • Serves as the central clinical communication hub, coordinating with physicians, clinicians, patients, families, and interdisciplinary partners to manage orders, labs, changes in condition, and transitions of care
  • Provides clinical oversight and quality assurance through documentation review, case conferences, utilization monitoring, risk reporting, and compliance with clinical, coding, and payer requirements
  • Provides clinical leadership and support, including staff guidance, orientation, direct patient care as needed, participation in quality improvement initiatives, and adherence to on call and episode management processes
