About The Position
Vironix Patient Care Coordinators use their clinical training to help patients stay healthier from home, without the physical demands of bedside care. They meet patients on-site at partner clinics to set them up with home health monitoring tools, then check in remotely by phone or video to review vitals and follow up on patients’ well-being. The role offers a flexible hybrid schedule that employees help shape weeks in advance. It is intended for Medical Assistants, LPNs, and RNs who want to continue using their clinical skills and providing patient care while having more control over how and where they work.

- Active certification is required.
Responsibilities
- Meet patients on-site at partner clinics and set them up with home health monitoring tools.
- Check in with patients remotely by phone or video, review their vitals, and follow up on how they are doing.
