About The Position

This role assists physicians and other healthcare providers in identifying patients who require aggressive disease management. The Patient Care Coordinator supports primary care and sub-specialty co-management through effective communication, follow-up, and integration of the patient's disease management plan. This position also assists the medical director in measuring patient outcomes and works with providers to redesign care processes and improve quality. Responsibilities include patient education related to specific chronic diseases, developing and implementing patient education sessions with providers, and facilitating patient access to care based on patient and family needs. Collaboration with electronic medical record team members to develop and manage disease-specific registries is also a key function. Other duties as assigned or requested.

Requirements

  • Education and licensing at a level of medical assistant.
  • 3-5 years experience in a clinical environment.
  • Experience in chronic disease management.

Responsibilities

  • Facilitates patient access to care based upon the needs of the patient and family. Works with the electronic medical record team members to develop and manage disease specific registries.
  • Performs other duties as assigned or requested.
