About The Position

The Patient Care Assistant (PCA) provides hands-on care, supervision, and rehabilitation support to adult patients recovering from neurological injuries or living with long-term care needs. Working under the supervision of licensed nurses and rehabilitation professionals, the PCA helps patients with daily routines, therapeutic activities, and behavioral support in a compassionate, structured environment designed to promote recovery and independence.

Nexus Neurorecovery Center – Conroe offers neurological rehabilitation for adults recovering from severe traumatic brain injury, stroke, spinal cord injury, and other neurological diagnoses, as well as long-term residential care for individuals with serious, life-altering injuries requiring ongoing clinical and behavioral support.

Requirements

• CPR/BLS certification is required.
• De-escalation training is required within 30 days of hire.
• Ability to commit to 12-hour shifts and flexibility to work days, nights, or weekends.

Responsibilities

• Provide direct care and assistance with activities of daily living, including bathing, dressing, feeding, mobility, transfers, and grooming.

• Support therapeutic, rehabilitative, and recreational programming to encourage patient engagement and progress.
• Assist in implementing individualized care and behavioral support plans under clinical supervision.
• Maintain a safe, organized, and therapeutic environment, following patient care protocols and safety procedures.
• Respond to crises or behavioral escalations using approved de-escalation and intervention techniques to maintain safety.
• Collaborate with nurses, therapists, case managers, and rehabilitation specialists to ensure coordinated and holistic care.
