Patient Care Assistant - Day Shift
About The Position
The Patient Care Assistant (PCA) provides hands-on care, supervision, and rehabilitation support to adult patients recovering from neurological injuries or living with long-term care needs. Under the supervision of licensed nurses and rehabilitation professionals, the PCA assists patients with daily routines, therapeutic activities, and behavioral support in a compassionate, structured environment designed to promote recovery and independence. Nexus Neurorecovery Center – Conroe serves adults recovering from severe traumatic brain injury, stroke, spinal cord injury, and other neurological diagnoses through its RISE Program. Its Life Care Program provides long-term residential care for individuals with serious, life-altering injuries who require ongoing clinical and behavioral support.
Requirements
- CPR / BLS certification is required.
- De-escalation training is required within 30 days of hire.
- Ability to commit to 12-hour shifts and flexibility to work days, nights, or weekends.

Responsibilities
- Provide direct care and assistance with activities of daily living, including bathing, dressing, feeding, mobility, transfers, and grooming.
- Observe, monitor, and accurately document patient behavior, participation, and vital signs.
- Support therapeutic, rehabilitative, and recreational programming to encourage patient engagement and progress.
- Assist in implementing individualized care and behavioral support plans under clinical supervision.
- Maintain a safe, organized, and therapeutic environment and follow patient care protocols and safety procedures.
- Respond to crises or behavioral escalations using approved de-escalation and intervention techniques to maintain safety.
- Collaborate with nurses, therapists, case managers, and rehabilitation specialists to ensure coordinated, holistic care.
