About The Position

Kindred Hospital Melbourne and Acute Rehabilitation Unit (ARU) is a 58-bed hospital offering extended recovery care. With 47 long-term acute care (LTAC) hospital beds, it partners with physicians to offer 24-hour clinical care seven days a week. The 11-bed ARU is designed for individuals recovering from debilitating effects of acute injury, impairment, or illness. The hospital is located off of NASA Boulevard on the outskirts of downtown Melbourne, near the Indian River and local amenities. The Patient Assessment Coordinator conducts patient assessments for potential admissions according to company standards. This role involves maintaining positive relationships with referral sources, sales teams, and internal departments. The coordinator is responsible for transmitting clinical and benefits information for potential admissions into ScionHealth Specialty Hospitals. Key duties include collecting, organizing, evaluating, and communicating clinical information effectively, demonstrating strong customer service, attention to detail, and a commitment to following established processes. The role requires operating with urgency for quick results and maintaining patient information confidentiality in compliance with regulatory requirements.

Requirements

  • Ability to collect, organize, evaluate and communicate pertinent clinical information with effective verbal and written skills.
  • Strong customer service skills and commitment to service excellence.
  • Computer skills that include Microsoft Office, Word, Excel and Outlook and PowerPoint.
  • Excellent presentation and communication skills.
  • Clinical Background with ability to conduct Patient Assessments.
  • Ability to work under stress and to respond quickly in crisis situations.
  • Knowledge of healthcare marketing and plan development.
  • Ability to work cooperatively as a member of a team.
  • Must read, write and speak fluent English.
  • Must have good and regular attendance.
  • Associate or Bachelor’s degree.
  • Two (2) years clinical patient care/treatment experience in healthcare field preferred.
  • Demonstrated understanding of Medicare, Medicaid, and other payers as related to LTACH benefits.

Nice To Haves

  • Degree in healthcare related field preferred.
  • Healthcare professional licensure preferred.
  • Registered Nurse, Licensed Practical Nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Social Worker license.

Responsibilities

  • Conducts patient assessments in accordance with company standards for appropriate patients for potential admission into the system.
  • Maintains positive relationships with referral sources, sales teams, admission, managed care, clinical and operational teams.
  • Coordinates transmission of clinical and benefits information of patients for possible admission into our ScionHealth Specialty Hospitals.
  • Collects, organizes, evaluates, and communicates pertinent clinical information with effective verbal and written skills.
  • Operates with a sense of urgency to produce quick results.
  • Maintains all patient information in a manner that ensures confidentiality and meets all regulatory requirements.
  • Conduct patient assessment in accordance with company standards.
  • Work closely with patients and their family members to educate them on the benefits of Kindred Hospital.
  • Assist in conducting tours of the hospital with families and referral sources as needed.
  • Facilitates the timely admission of patients from the referral sources: Identifies medical record information needed from referring facility; Completes the Pre-Admission Clinical Evaluation (PACE) form to assist in evaluating appropriateness of admission; Facilitates and coordinates physician-to-physician communication as necessary; Identifies specific patient needs for equipment, supplies and consult services.
  • Meets with management staff, participates in marketing meetings and serves on various Internal committees as assigned.
  • Maintains a state-of-the-art knowledge of areas of responsibility and develops and follows a program of continuing education.
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
