In this role, you will support individuals in six state correctional institutions throughout the Fox Valley. Using Dr. Marcella Wilson’s Transition to Success model and the Map of My Dreams workbook, this role helps participants identify goals, overcome barriers, and prepare for successful reentry. Responsibilities include providing pre-release case management, connecting individuals to community resources, and supporting education, employment, and career planning after release. Success in this role requires experience motivating, teaching, and guiding individuals toward post-secondary education and workforce opportunities.

Requirements

  • Experience motivating, teaching, and guiding individuals toward post-secondary education and workforce opportunities.
  • Associate’s degree or equivalent work experience
  • Minimum of two years’ experience in a related field
  • Excellent communication, organizational, and interpersonal skills
  • A genuine commitment to helping others succeed

Responsibilities

  • Meet with individuals in 6 state correctional institutes to share information about Pathways Home and complete program enrollment.
  • Provide case management services to assist individuals in achieving more effective personal, educational and/or vocational outcomes.
  • Responsible for achieving specific enrollment benchmarks within the required target population to achieve and exceed program performance standards.
