About The Position

The Re-Entry Coordinator develops, coordinates, and evaluates re-entry programming and continuity-of-care services that support incarcerated patients transitioning back into the community. This role builds linkages between the facility and community-based organizations, primary care, behavioral health, and substance use treatment providers, with emphasis on connecting patients to MAT/MOUD and other services to support successful reintegration and reduce recidivism. Work is performed under general supervision in collaboration with clinical and custody teams.

Requirements

  • Bachelor's degree in social work, criminal justice, public administration, human/behavioral sciences, or a related field preferred; equivalent experience may substitute.
  • Minimum of two (2) years of experience in re-entry, social services, case management, or a related field.
  • Knowledge of the criminal justice system, re-entry principles, and evidence-based practices.
  • Knowledge of community resources and ability to establish partnerships with organizations and providers.
  • Strong organizational, communication, and interpersonal skills; ability to work independently and as part of a team.

Nice To Haves

  • Experience working with justice-involved and diverse populations in a corrections or reentry setting.
  • Familiarity with MAT/MOUD continuity-of-care coordination.
  • Licensed clinician credential a plus.

Responsibilities

  • Plan, coordinate, perform, and evaluate re-entry programming and services related to community reintegration, serving as a case manager where appropriate.
  • Develop and implement written procedures to provide program services.
  • Develop and maintain linkages and referral relationships between the facility and community-based organizations, primary care, behavioral health, and substance use providers.
  • Refer and connect clients to community resources to continue care after release, including MAT/MOUD continuity.
  • Conduct assessments to determine individual needs and develop individualized re-entry plans.
  • Monitor and track participant progress and adjust services as needed to support success.
  • Coordinate discharge planning and continuity-of-care with nursing, behavioral health, and medical providers.
  • Provide guidance and support to help individuals navigate the challenges of re-entry.
  • Maintain accurate and complete records of all program activities demonstrating compliance.
  • Review and analyze activity reports and statistics to evaluate program effectiveness and recommend improvements.
  • Develop, implement, and review procedures to ensure compliance with regulations and standards.
  • Maintain confidentiality and adhere to Thrive Medical Services policies, NCCHC standards, and applicable laws.
