The Transitional Living Program (TLP) is a voluntary residential program serving young people ages 16-21 who are experiencing homelessness or are at imminent risk of homelessness. The TLP can house up to 6 youth. The goal of the program is to promote independence, self-sufficiency, and empowerment. To achieve this, youth in the program are required to participate in five program domains: therapy, case management, life skills training, employment, and volunteer. Residents may live at the TLP for up to 18 months, depending on age, their individual discharge plan, and their participation within the program. All participants receive aftercare supports upon exit. The TLP is open 24 hours per day, 365 days per year. The TLP is the only program of its kind in Macomb County. Transitional Living Specialists (TLS’) provide trauma informed care, supervision, and guidance to up to 6 residential youth.
