About The Position

Affinity Hospice is a growing hospice organization dedicated to providing exceptional care to patients and their families. The Palliative Support Specialist serves as the primary operational point of contact for the palliative care team, managing essential administrative tasks such as intake, scheduling, insurance verification, non-clinical order coordination, and communication to allow clinicians to focus on patient care. This is a non-licensed, administrative role.

Requirements

  • High school diploma required
  • 1+ year in healthcare operations/medical office
  • Strong customer service and phone etiquette
  • Accurate data entry
  • Basic spreadsheet/reporting skills
  • Ability to manage multiple queues
  • Sound judgment in routing/escalation
  • Familiarity with EMR, e-signature, payer eligibility, and collaboration tools (training provided)

Nice To Haves

  • Associate's degree preferred
  • Palliative/hospice experience preferred

Responsibilities

  • Serve as the first point of contact for patients/families, managing calls and messages, resolving routine needs, and coordinating appointments.
  • Monitor incoming referrals, create/maintain patient records in the EMR, and ensure required documentation is captured and filed accurately.
  • Complete insurance eligibility/benefit checks and document results; collaborate with revenue cycle on payer requirements and authorizations.
  • Prepare and route order packets for signatures; coordinate with internal/external partners for service and equipment fulfillment; track status.
  • Support record requests, consents, and disclosures in accordance with policy, ensuring privacy, accuracy, and timely transfer of information.
  • Maintain clear, timely updates in shared workspaces to support handoffs among clinical, sales, and leadership teams; escalate barriers and exceptions.
  • Maintain standardized logs and update designated data fields to support routine reporting, throughput visibility, and audits.
  • Adhere to HIPAA, state/federal, accrediting-body, and internal policy requirements; participate in simple chart checks and process improvements.
  • Provide general office support, coverage for peers, and assist with testing or updates to approved systems and workflows.
