The Palliative Care Social Worker provides psychosocial assessment and evidenced based psycho-social intervention for palliative care patients and their families. A member of the Palliative Care interdisciplinary team, Palliative Care Social Workers utilize their skills and knowledge of person-centered system based clinical skills to alleviate stress associated with serious illness and end of life care. Palliative Care Social Workers provide specialty bereavement and grief interventions to patients, families, and staff across the healthcare system.

Requirements

  • Master's Degree from an Accredited School of Social Work
  • PA Social Worker License

Nice To Haves

  • 2 years of experience providing Palliative Care or End of Life care
  • 2 years of social work experience in a medical setting
  • PA Clinical Social Worker License

Responsibilities
