About The Position

The Palliative Care Social Worker provides psychosocial assessment and evidenced based psycho-social intervention for palliative care patients and their families. As a member of the Palliative Care interdisciplinary team, they utilize their skills and knowledge of person-centered system based clinical skills to alleviate stress associated with serious illness and end of life care. This role also involves providing specialty bereavement and grief interventions to patients, families, and staff across the healthcare system.

Requirements

• Master's Degree Accredited School of Social Work

Nice To Haves

• 2 years of experience providing Palliative Care or End of Life care
• 2 years of social work experience in a medical setting
• PA Social Worker License
• PA Clinical Social Worker Lic
