About The Position

IN Compass Health is building its hospital-based palliative care service at AnMed Medical Center in Anderson, South Carolina. AnMed is a 461-bed, not-for-profit hospital serving the Upstate South Carolina area since 1908. Our palliative care team aims to provide providers with the time and support needed for meaningful conversations and comprehensive care. This role offers a unique approach to palliative care, emphasizing quality of care over quantity of encounters, and fostering a truly interdisciplinary team environment.

Requirements

• Active South Carolina licensure as an NP (FNP, AGNP, or AGACNP) or PA-C
• DEA registration (or eligibility)
• Strong communication skills, especially around serious illness and end-of-life conversations
• Comfort working autonomously within a collaborative team

Responsibilities

• Complex symptom management
• Goals-of-care conversations with patients and families
• Code status discussions, advance care planning, and POLST/MOLST completion
• Family meetings, including those involving complex dynamics or surrogate decision-making
• Coordination of transitions to home hospice, inpatient hospice, or palliative-supported discharge
• Prognostic support and education for primary teams
