Requirements

Graduate of an approved school of nursing.
Current license as a practical nurse in the State of North Carolina.
Two years' experience required.
EPIC experience required.

Nice To Haves

Office/clinic experience preferred.
Palliative care experience preferred.
Oncology experience preferred.

Responsibilities

Provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
Collaborate and coordinate with external medical staff and referral sources, as well as internal clinical teams for continuity of care.
Assist field staff with, or independently complete, prior authorizations for medications prescribed by palliative care.
Assist with rooming patients, including vital signs, medication reconciliation, basic assessment of chief complaint, and related documentation.
Call patients for symptom checks, assist patients with refills, perform phone triage, and address other patient questions or concerns.
Educate patients and caregivers on identifying signs and symptoms, medication information, and plans of care.
Be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and medical social worker.
Be cross-trained with clinic scheduling and other administrative tasks.
