Requirements

  • MSW from a CSWE accredited School of Social Work.
  • Licensed Clinical Social Work license required.
  • At least two years of professional social work experience, preferably in a health care setting.

Nice To Haves

  • Completion of American Cancer Society Leadership in Oncology Navigation / PIN training and credentialing within one year of hire.

Responsibilities

  • Provide comprehensive assessment of social, emotional, and practical needs to patients and families in the outpatient PC clinic, private residences, and care facilities via in-person or telehealth.
  • Introduce and educate patients and families about Palliative Care and Hospice services.
  • Conduct independent psychosocial assessments/evaluations.
  • Provide 1:1 psychotherapy as a reimbursed service.
  • Participate in professional team reviews of patient care and cases.
  • Provide casework and group work for patients and their families.
  • Identify communication resources and assist patients and families in connecting with resources.
  • Facilitate advance care planning.
  • Perform bio-psychosocial assessments to identify emotional, social, spiritual, financial, and environmental strengths/stressors related to diagnosis, illness, treatment, and life situation.
  • Conduct supportive counseling and problem-solving within the scope of degree/license to address issues related to chronic/terminal illness.
  • Facilitate goals-of-care meetings and counseling sessions/visits to address immediate needs/concerns.
  • Provide comprehensive outpatient psychosocial evaluation and therapy, including diagnostic interviews and psychotherapy.
  • Collaborate with the palliative care team in identifying and achieving patient care goals.
  • Coordinate care with other LCFL staff, hospital providers/staff, and external agency staff.
  • Maintain working knowledge of community resources and educate patients, families, and staff on accessing and utilizing them.
  • Attend/participate in continuing development annually to enhance professional practice.
  • Appropriately communicate information in accordance with LCFL policies and procedures.
  • Organize and perform work effectively and efficiently, including documentation and billing for psychotherapy services.
  • Maintain and adjust schedule to enhance program performance.
  • Demonstrate positive interpersonal relations.
  • Maintain and promote customer satisfaction.
  • Effectively demonstrate the mission, vision, and values of Lower Cape Fear LifeCare daily.
