About The Position

This role is responsible for coordinating palliative care services, managing patient information, and collaborating with various healthcare professionals and insurance providers. The Palliative Care Coordinator ensures smooth transitions of care, accurate documentation, and efficient clinic operations while maintaining strict patient confidentiality.

Requirements

  • High school diploma.
  • Certified Medical Assistant or graduate of an accredited school of practical nursing and licensed as an LPN through the Ohio Board of Nursing.
  • Minimum of 1 year working in a healthcare office environment with coordinating responsibilities.
  • Experience working with healthcare insurance.
  • Strong data entry and computer skills, including electronic medical records, Microsoft Word, Excel, and Outlook.
  • Demonstrated ability to effectively manage time, multi-task, and meet deadlines.
  • Strong verbal and written communication skills, customer service, and phone skills.
  • Ability to work independently and within an interdisciplinary team.
  • Demonstrates commitment to the philosophy, goals, and objectives of hospice and palliative care.

Responsibilities

  • Respond to incoming calls from patients, families, and providers, directing them to the appropriate team member and ensuring timely documentation.
  • Collaborate with hospital partners and team members to manage care transitions and communicate vital information.
  • Schedule patient consult and follow-up visits in coordination with provider scheduler and clinic hours.
  • Conduct insurance verifications and complete prior authorizations, entering information into the EMR.
  • Maintain the prior authorization log, ensuring extensions are completed in a timely manner.
  • Accurately enter referral, admission, insurance, contact, billing, and other required information into the electronic medical record system within established timeframes.
  • Assist in the clinic to facilitate patient care and daily operations as needed.
  • Learn and utilize agency-provided technology to increase efficiency, communication, and performance.
  • Attend staff meetings and complete all mandatory training.
  • Maintain strict confidentiality of all patient information, disclosing it only to authorized individuals.
  • Perform other duties as assigned.
