About The Position

The Palliative Care Coordinator is responsible for managing incoming calls from patients, families, and providers, ensuring timely documentation, and collaborating with hospital partners and team members to manage care transitions. This role involves scheduling patient visits, conducting insurance verifications, completing prior authorizations, and accurately entering patient information into the electronic medical record system. The coordinator will also support clinic operations, utilize agency technology, attend staff meetings, and maintain patient confidentiality.

Requirements

  • High school diploma.
  • Certified Medical Assistant or graduate of an accredited school of practical nursing and licensed as an LPN through the Ohio Board of Nursing.
  • Minimum of 1 year working in a healthcare office environment with coordinating responsibilities required.
  • Experience working with healthcare insurance also required.
  • Strong data entry and computer skills working within electronic medical records, Microsoft Word, Excel and Outlook.
  • Previous experience must demonstrate the ability to effectively manage time, multi-task and meet deadlines.
  • Must demonstrate strong verbal and written communication skills, as well as customer service and phone skills.
  • Is a self-starter with the ability to work independently and within an interdisciplinary team.
  • Demonstrates commitment to the philosophy, goals and objectives of hospice and palliative care.

Responsibilities

  • Responds to incoming calls from patients, families and providers and directs to the appropriate team member; ensures required documentation is completed in a timely manner.
  • Collaborates with hospital partners and other team members to manage care transitions and communicate vital information.
  • Schedules patient consult and follow-up visits in coordination with provider scheduler and clinic hours.
  • Conducts insurance verifications, completes prior authorizations, and enters information into EMR.
  • Maintains prior authorization log ensuring extensions are completed in a timely manner.
  • Accurately enters referral, admission, insurance, contact, billing and other required information into the electronic medical record system within established timeframes.
  • May be assigned responsibilities in the clinic to help facilitate patient care and daily operation.
  • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication and performance of duties.
  • Attends staff meetings and completes all mandatory training.
  • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
  • Performs other duties as assigned.
