About The Position
The Palliative Care Coordinator is an integral part of the team assisting providers to provide excellent care, setting the standard in our community.
Requirements
- At least 18 years of age
- Ability to read and follow written instructions and document care given
- Satisfactory references from previous employers
- Is self-directing with the ability to work with little direct supervision
- Good oral and written communications
- Good organizational skills
Responsibilities
- Receive, process and coordinate referrals and following up with patient in a timely manner to coordinate admission
- Scanning and upload patient documents
- Assure orders are sent to physicians in a timely manner
- Handling incoming calls and other communications
- Maintain organization of the office common area
- Obtains and documents prior authorization for services from insurance providers
- Schedule providers to see patients on a recurring basis
- Assists to maintain liaison relationships with hospitals, facilities, insurance personnel, providing information and education on organization services, coverage issues and related areas.
- Resolve appointment conflicts for staff
- Produce and process schedules and reports as they are requested by administrative staff
- Assure admit packets are stocked and ready for both admissions
- Assist with claim denials
