About The Position

Roze Room is a leading provider of Hospice and Palliative Care, celebrating over 25 years of service to Southern California communities. Seeking a Full Time Palliative Care Coordinator. This is an in-office position located in Pasadena, CA, with working hours from Monday to Friday, 9am-6pm. The salary range is $21.00 - $25.00/hr, based on experience. The ideal candidate must be computer savvy, possess excellent customer service skills, and have Palliative Care/Hospice experience. Spanish speaking is preferred. We are looking for someone with strong computer and customer service skills, excellent communication and organizational abilities, and experience in hospice, home health, or palliative care. The ability to multitask in a fast-paced healthcare environment and a professional, compassionate, and team-oriented attitude are essential. A valid driver's license and reliable transportation are also required. Key responsibilities include coordinating palliative care referrals and admissions, obtaining insurance authorizations and re-authorizations, scheduling patient visits and coordinating care services, communicating with physicians, nurses, social workers, and care teams, reviewing documentation for accuracy and timely submission, supporting patient and family needs while ensuring quality care delivery, and maintaining accurate patient records and reports.
