About The Position

This role provides case management for an assigned group of clients, coordinating and monitoring the activities of the individual treatment team (ITT). The PACT Registered Nurse assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the ITT. Responsibilities include providing individual supportive therapy and symptom management, ensuring immediate changes are made to treatment plans as client needs change, educating and supporting clients' families, and advocating for clients' rights and preferences. The role also involves conducting comprehensive assessments of psychiatric history, consulting with community agencies and families for treatment coordination, and providing on-call crisis intervention during regular hours, serving as a backup to other staff. Documentation of client progress is essential for maintaining permanent records. The nurse will participate in daily staff meetings and treatment planning reviews, and may take a lead role or participate in medication administration and medical services. Under the direction of the team psychiatrist, the nurse will develop, revise, and supervise team psychopharmacologic and medical treatment policies, evaluate and chart psychotropic medication effectiveness, complications, and side effects, and arrange for required lab work. They will also organize and manage the system for medication delivery to clients, integrate medication administration into treatment plans, and manage pharmaceuticals and medical supplies. In collaboration with the team psychiatrist, the nurse will coordinate and administer medical assessments of client physical health and treatment, and participate in treatment, rehabilitation, and support services. Ongoing assessment of clients' mental illness symptoms and treatment response, as well as providing symptom education, are key duties. The role may involve assisting with daily living activities and performing medication delivery, working rotating weekdays, weekends, and holidays. Passing a medication test within 30 days of hire and maintaining RN licensure are required. The nurse may also be called upon to speak to community groups.

Requirements

Must be licensed as a registered nurse
At least one year of work experience in an inpatient or outpatient psychiatric setting
Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences’ work in market jobs’ and have access to helpful, adequate, competent, and continuous supports and services
Must possess the skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment
Must pass medication test within 30 days of hire
Must pass Background Checks and Motor Vehicle Record Checks

Nice To Haves

Responsibilities

Evaluates and charts psychotropic medication effectiveness, complications, and side effects
Arranges for required lab work according to protocol
Under the direction of the team psychiatrist organizes and manages the system of getting medication to clients and integrating medication administration tightly into clients’ individual treatment plans
Under the direction of the team psychiatrist manages pharmaceuticals and medical supplies
In collaboration with the team psychiatrist, coordinates, schedules, and administers medical assessments of client physical health and treatment
Participates in treatment, rehabilitation, and support services
Provides ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment
Provides symptom education to enable clients to identify their mental illness symptoms
Assists with daily living activities
Performs medication delivery
Works rotating weekdays, weekends, and holidays
Maintains RN license
May be called upon to speak to community groups
