About The Position

Aptive Resources, in partnership with Valor Healthcare, is hiring a Primary Care Physician (MD/DO) to deliver comprehensive primary care to Veterans within the Patient Aligned Care Team (PACT) model at the proposed Yreka, CA Community-Based Outpatient Clinic (CBOC), pending VA contract award. The role involves providing health care commensurate with licensure, clinical privileges, and scope of practice, ensuring care plans contain clinically indicated medical recommendations, and offering all clinically indicated services to PACT-assigned patients. The physician will lead the PACT team, delegating care processes, reviewing clinical and performance data for continuous improvement, and ensuring patients have same-day access for both face-to-face and virtual care visits during clinic hours. Performance benchmarks include meeting greater than 85% of same-day appointment demand and closing greater than 95% of encounters in the VA electronic health record (CPRS) within 72 hours.

Requirements

  • Evidence of education, training, and experience in Internal Medicine or Family Practice
  • Board certified (or board eligible) by the American Board of Medical Specialties (ABMS) or the Bureau of Osteopathic Specialists (BOS) in Internal Medicine and/or Family Practice
  • Authorization for prescriptive authority required
  • Current, full, active, and unrestricted license to practice medicine in the state where the clinic is located, or otherwise meets VA Qualification Standards for Physician
  • Must hold a current, full, active, and unrestricted license/certification in California, or otherwise meet VA Qualification Standards for the position.
  • Must successfully complete VA credentialing/privileging review and a background investigation (cleared through OPM via the VA Security and Investigations Center) prior to starting work; PIV credentialing required.
  • Must complete required VA mandatory training and maintain current licensure/certification documentation annually.

Responsibilities

  • Provide health care commensurate with licensure, clinical privileges, and scope of practice to assigned and unassigned patients presenting for care
  • Ensure the patient's care plan contains clinically indicated medical recommendations and offer all clinically indicated services to PACT-assigned patients
  • Provide leadership to the PACT team, including shared delegation of appropriate care and care processes to other team members
  • Review available clinical and performance data with the team and focus on continuous improvement of critical team processes
  • Ensure patients have same-day access for face-to-face and virtual care visits during regular clinic hours
  • Collaborate with PACT staff to develop personal health plans incorporating care management and coordination appropriate to patient needs
  • Meet performance benchmarks of greater than 85% same-day appointment demand met and greater than 95% of encounters closed in the VA electronic health record (CPRS) within 72 hours
