About The Position

The purpose of the PACT (Pitkin Area Co-Responder Teams) program is to support law enforcement on calls for service involving mental health and/or substance use and, when possible, divert individuals with behavioral health disorders or symptoms from involvement in the criminal justice system, engaging them in treatment and/or social services instead. The PACT case manager will primarily work with a diverse population including individuals who are struggling with mental health issues, substance use, and/or are involved in the criminal justice system. Patient population could include children, adults, individuals, couples, and groups. The case manager connects individuals and families to a wide range of resources and coordinates therapeutic and community-based services. The case manager may also assist with transportation as needed.

Requirements

  • Bachelor's degree required; preferably in human services or related field.
  • Bachelor's degree in a non-related field may perform the functions of this position only if they have one year experience in a behavioral healthcare field.
  • CPR/BLS Certification for Healthcare Provider prior to independent patient care required.
  • Working knowledge of Zoom, Teams and/or other tele-health platforms and applications.
  • Ability to produce written documentation of clinical work in a manner readily understood by other service providers.
  • Ability to summarize service themes from a variety of client contacts and communicate suggested improvements for service delivery.
  • Ability to demonstrate appropriate clinical judgment when assessing the level of risk and determining resolution of client emergencies, resulting in a safe outcome.
  • Working knowledge of Windows based computer applications to include Microsoft Word, Microsoft Excel, Microsoft Outlook, and Internet access.
  • Ability to operate standard office equipment including computer keyboard, calculator, copy machine, fax and multi-line telephone.
  • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
  • Must already be authorized to work in the US; sponsorships not available.

Nice To Haves

  • Previous experience with substance abuse and mental health-related case management and treatment preferred.
  • Minimum of two (2) years human service related experience preferred.
  • Certified Addiction Specialist is strongly preferred.
