About The Position

Under the supervision of the PACE Therapy Technical Manager, the PACE Therapy Supervisor oversees site-specific San Diego PACE therapies (physical therapy, occupational therapy, speech therapy, and related) service provision, consistent with standards and professional scope of each specific therapy discipline. This position is responsible for leading, training and developing therapists from each discipline and associated paraprofessional staff. This leader will ensure high quality, effective and accessible services are available to PACE clients. This position is responsible for ensuring fluency and administrative oversight of daily operations within the assigned Therapy Department, which includes professional and para-professional staff, in the performance of their job duties, ensuring accurate and timely documentation in participant electronic medical record. The PACE Therapy Supervisor may, as needed, carry a case load (panel), and will train therapists and aides/assistants to effectuate therapies services, including planning, organizing, implementing and closing the loop for all therapies services, towards achieving highest functionality and quality of life for San Diego PACE participants. The PACE Therapy Supervisor will oversee outcomes for the department and provide weekly quality and productivity reports to the Site Manager and Therapy Technical Manager. Responsibilities include but are not limited to: organization of daily workflow, supervision of therapy department employees and related client outcomes, QAPI projects, assessments, auditing charts, managing employee performance, therapeutic modalities/treatment(s), teaching/educating participants and/or their legal representative/caregiver. The Therapy Supervisor coordinates overall daily operations, and serves as a liaison between the interdisciplinary team, clients/caregivers/legal representatives, direct care staff, Therapy Manager and community agencies.

Requirements

  • Successful completion of any of the following programs: PT, OT, SLP, PTA, COTA via an accredited academic institution.
  • Minimum of 1 year experience working in a therapy department.
  • Certificate in rehabilitation therapy or related field (PT, OT, SLP, PTA, COTA).
  • First Aid/CPR Certification.
  • Valid California driver’s license; reliable transportation with proper insurance to meet San Diego PACE guidelines.
  • Excellent oral, written communication, and interpersonal skills.
  • Thorough understanding of the Scope of Practice for” Occupational Therapy, Physical Therapy, Speech Therapy, Physical Therapy Assistant and Certified Occupational Therapists, including knowledge of various therapeutic techniques and modalities.
  • Ability to effectively and efficiently utilize EPIC EMR and other documentation outlets.
  • Thorough knowledge of therapy department processes.
  • Organization and project management skills.
  • Ability to establish and maintain good interpersonal relationships.
  • Ability to work independently and meet deadlines.
  • Comfort performing, home, hospital, SNF, etc. visits, with frequent travel in San Diego county.
  • Knowledge of community resources and appropriate utilization to meet the needs of clients and employees.
  • Customary Therapy Department equipment and DME, Office equipment, including but not limited to computers, phones (including cell phones), copy machines, fax machines.
  • Prolonged periods of sitting and standing.
  • Frequent walking.
  • Ability to remain calm and objectively evaluate problems.
  • May be required to work outside of normal working hours.
  • Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs; ability to lift and transfer participants; T.B. clearance; must maintain current First Aid and CPR; must have reliable private vehicle, proof of CA driver’s license and maintain proof of current vehicle insurance.
  • Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
  • Compliance with all mandated vaccinations and all boosters is a term and condition of employment.

Responsibilities

  • Responsible for the overall provision of direct therapies services to the participants of the San Diego PACE program, ensuring adherence to policies and procedures of PACE, applicable laws and licensing requirements at all times.
  • Communicates job expectations with employees and managing employee performance through coaching, counseling, and disciplining and conducts performance evaluations for the therapy team.
  • Oversees the development of therapists and related paraprofessionals, including monitoring and feedback of all aspects of services provided to individuals.
  • Participate in the hiring process, train and develops therapists and related paraprofessionals.
  • Train therapists to conduct robust assessments, care planning and periodic re-assessments (minimally every 6 months) of each participant's case.
  • Ensures Therapy Disciplines actively engage and contribute meaningfully during Interdisciplinary Team meetings.
  • As assigned, conduct robust assessments, care planning and periodic re-assessments (at least every 180 days) of participant's cases.
  • Ensures assessments, and care plans are thorough and completed prior to the scheduled team meeting, including communicating participant changes to team members.
  • Ensures that all appropriate therapists complete Plan of Care signing and documentation in PACE Electronic Health Record (EHR) system
  • Be proficient in PACE Electronic Health Record (EHR) system and in collaboration with the IT Department, ensure the initial staff training and on-going proficiency on and appropriate use of PACE EHR.
  • Provide superior customer service to stakeholders, and team members.
  • Appropriately escalate issues/concerns to Therapy Technical Manager
  • Consults with, develops and advises staff members as to the interconnectedness of movement and physical limitations, and it’s impact on health.
  • Fosters a positive and professional environment of effective relationship building amongst staff and maintain a cohesive team.
  • Participates in Program and Policy development of Therapy department.
  • Conducts home visits as necessary to observe and treat clients, in compliance with scope of practice and care needs of clients.
  • Coordinate necessary DME and therapies services as necessary, in conjunction with interdisciplinary team, including communication with attending physician.
  • Communicates with family, legal representatives and caregivers frequently and as appropriate.
  • Acts as a resource for education and training to other team members and day center staff regarding topics such as DME needs/use, physical limitations and client status changes.
  • Ensures that all participants complete Care Plan signing after every assessment.
  • Prepares work schedules and ensures employees have the resources necessary to succeed, including resources to provide appropriate referrals to community agencies or facilities; acts as liaison with such organizations and as an advocate for participants.
  • Assigns & monitors therapies caseloads for proper service delivery, quality and compliance.
  • Ensures required documentation is in place, accurate, up-to-date, and in compliance with internal and external regulations and policies.
  • Acts as site program administrator during absence of Therapy Technical Manager.
  • Resolves employee issues within position responsibilities.
  • May work on special assignments in addition to normal job functions, including traveling to other San Diego PACE sites.
  • Enhances professional growth and development through participation in educational programs current literature, in-service meetings and workshops.
