About The Position

The Outreach Worker will be responsible for providing comprehensive assessments, planning, implementation and overall evaluation of individual customer needs and intake. Plan, organize, or coordinate the activities of Gateway’s community outreach programs in assigned counties. Seeks ways to improve service delivery. Assesses services from the customer’s point of view. Emphasizes a team approach to providing great customer service. Shows commitment/dedication and accountability in one’s work, and follows through on all projects, goals, aspects of one’s work. Shows willingness to learn new methods, procedures, or techniques, resulting from departmental or agency-wide change. Recognizes key actions and underlying issues and problems Masters new technical and organizational concepts and information Sets priorities with an appropriate sense of what is most important and plans with an appropriate and realistic sense of the time demand involved. Identifies the information needed to solve a problem effectively. Works to develop and maintain positive working relationships with co-workers by being punctual, keeping personal telephone calls to a minimum, and maintaining a pleasant work attitude. Promotes safety within the work environment and agency. Advocate on behalf of customers who lack essential education and communication skills, and the ability to provide basic needs for themselves and/or families. Provide case management, follow-up, home visits, representation , and other supportive services for clients. Work with other agency staff and community partners to coordinate existing activities and meetings in each county. Establish and maintain a cooperative and effective working relationship with community partners, clientele, fellow employees, local government(s), and all other agencies who are serving the same clientele. Create and maintain a system of record keeping and maintain an organized filing system of customer case files and other necessary documentation that must be legible, chronological and stored in a secured location to ensure customer confidentiality. Transport target board members to the semi-monthly board meetings, if necessary.

Requirements

  • Minimum qualifying education is a high school diploma or GED.
  • Valid Kentucky driver’s license, reliable transportation, and satisfactory background checks.
  • Pass a pre-employment drug test and random drug testing as required.
  • State required minimum vehicle liability insurance and uninsured motorist insurance.
  • Any/all other testing or applicable checks required by local, state, or federal law.

Nice To Haves

  • A bachelor’s degree from an accredited university or college with a major in a human service field or related study.
