About The Position

The Outreach Worker provides peer support, harm reduction services, recruits new patients, and participates in education and awareness-raising activities with individuals and groups. The Outreach Worker reports directly to the Sr. Director of Behavioral Health (Harm Reduction), who shall determine the responsibilities for this position. These responsibilities may be revised, as necessary, with proper notification and training provided, where appropriate.

Requirements

  • Personal or professional experience with vulnerable populations required
  • High School diploma or GED preferred
  • Peer certification preferred
  • awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

  • Personal or professional experience with the communities serviced (including people who use drugs, people living with HIV or HepC, and/or other vulnerable communities)

Responsibilities

  • Delivers harm reduction services, particularly sterile supply provision, Narcan, Overdose prevention education Hepatitis C education, and treatment options; these services may be provided at Evergreen’s downtown Buffalo location, Evergreen’s harm reduction mobile unit, or in community settings
  • Assists the Mobile Unit Coordinator on the mobile unit at designated sites
  • May be asked to assist with tabling events, festivals and community meetings
  • Works in collaboration and links patients to Peer Outreach Worker
  • Conducts outreach activities in the community, particularly in areas with a high prevalence of people who use drugs
  • Conducts syringe sweep activities, independently or as part of a team of employees, in areas where syringe litter is common
  • May be asked to provide assistance at the SSP front desk, in the Hub or Drop-In, and to engage with clients at that location
  • Solicits input from people directly impacted by substance use and mental health
  • Contributes to planning to strengthen and expand harm reduction services
  • Actively participates in routine supervision and team meetings as requested
