About The Position

Be the person who helps someone come in from the cold. For people experiencing homelessness, the hardest part is often not finding accommodation. It's finding someone who won't give up on them. At Change Grow Live, we believe everyone deserves the opportunity to live a healthier, safer and more fulfilling life. Every day, we work alongside people facing some of life's most complex challenges, helping them build stability, improve wellbeing and create positive futures. We're looking for a Homelessness Outreach Worker to join our North Northamptonshire team, based in Wellingborough, supporting people experiencing rough sleeping, homelessness, substance misuse and multiple disadvantage. This is an opportunity to be part of a service that doesn't wait for people to walk through the door. We go out into communities, build trust, create opportunities and help people take the next step towards lasting change. Homelessness is rarely just about housing. Many of the people we support are experiencing a combination of substance misuse, poor physical and mental health, social isolation, trauma, poverty and exclusion. As a Homelessness Outreach Worker, you'll work directly with people who are rough sleeping or at risk of rough sleeping, helping them access support, reduce harm and move towards safer, more settled lives. You'll build relationships with people who may have disengaged from services, advocate on their behalf and help them navigate pathways into accommodation, treatment, healthcare and wider community support. Working alongside local partners, housing providers, health services and community organisations, you'll help create coordinated responses that focus on people's strengths, aspirations and long-term outcomes. Most importantly, you'll ensure the people we support are heard, respected and empowered to make sustainable changes in their lives.
