About The Position

Street Health Outreach Medical Assistant Street Health Program At Father Joe’s Villages, Street Health meets people where they are - in the places most systems don’t reach. Our Street Health team provides outreach, engagement, and integrated healthcare support for individuals experiencing homelessness across San Diego County, helping people improve health outcomes and take meaningful steps toward stability and housing. As a Street Health Outreach Medical Assistant, you will play a unique hybrid role that combines, field-based outreach, care coordination support, and clinical medical assistant duties. You will meet patients in the community, build trust over time, and connect individuals to the healthcare system while supporting ongoing care in both field and clinic settings.

Requirements

  • High School Diploma or GED required.
  • Medical Assistant Certificate required.
  • Minimum of one (1) year of experience in outreach, healthcare, case management, care coordination, or related direct service work with underserved populations.
  • Basic computer proficiency and ability to complete documentation in electronic systems.
  • Valid California driver’s license.
  • Knowledge of medical assistant practices and clinical support procedures.
  • Understanding of trauma-informed care, harm reduction, ECM, and whole-person care models.
  • Strong documentation skills and comfort using electronic health records.
  • Ability to manage an assigned caseload across field and clinic settings.
  • Strong interpersonal skills with the ability to build trust and engage diverse populations.
  • Ability to work independently in the field and collaboratively within multidisciplinary teams.
  • Commitment to delivering services with compassion, respect, empathy, empowerment, and dignity.

Nice To Haves

  • Experience working with individuals experiencing homelessness, behavioral health conditions, substance use disorders, or complex health and social needs preferred.
  • Experience providing ECM services preferred.

Responsibilities

  • Conduct street-based and community outreach to engage individuals experiencing homelessness and connect them to healthcare and support services.
  • Maintain an assigned caseload, including Enhanced Care Management (ECM) members, to support ongoing engagement, follow-up, and continuity of care.
  • Provide clinical support including taking vital signs, documenting patient information, and assisting licensed clinicians with patient care activities.
  • Support care coordination across healthcare, housing, behavioral health, and social service systems in collaboration with interdisciplinary teams and community partners.
  • Complete accurate, timely documentation for outreach, clinical services, and ECM-related activities in electronic health records.
