About The Position
The Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and are determined to require a high level of service needs. The Outreach Therapist provides assessment and treatment interventions consistent with the program model, conducts clinical and risk assessments, and collaborates with individuals to create individualized, person-centered treatment plans. The role requires competency in age-related or specialty issues and developmental needs for each population served. Therapists work with clients to develop appropriate discharge plans utilizing natural support, maintain collaborative relationships with family members and other service providers, and provide crisis assessment and intervention to monitor the status of clients in crisis. Care may be provided in office, home, residential, and/or community-based settings. The position is responsible for maintaining client and staff records according to agency policy, providing coverage for team members during periods of absence, and participating in weekly supervision, consultations, and meetings. Therapists may transport clients using agency vehicles and/or personal vehicles and participate in all required CHR training and obtain/maintain certification in an EBP if applicable. In residential programs, therapists prioritize safety and physical presence, participating as full members of the milieu. Additional responsibilities may be required.
Requirements
- Master’s Degree or higher in related field.
- Experience working with trauma, mental illness and/or serious emotional disorders and/or substance use disorders preferred.
- LMSW, LPC-A, LMFT-A required.
- Valid Driver’s License for programs where transportation is required.
Nice To Haves
- LCSW, LMFT, LPC, Licensed Psychologist preferred.
- LADC may be substituted for LCSW, LMFT, LPC or Licensed Psychologist in some programs.
- Other training may be required based on program.
