Registered Nurse - Assertive Community Treatment Team
About The Position
Registered Nurses provide medical and psychiatric support within a person's community, focusing on clients experiencing mental illness who may lack access to adequate healthcare. This role involves care planning, assessment, and implementation as part of a client's recovery journey. The nurse collaborates with clinical teams, including psychiatrists and medical doctors, and provides health education. Key responsibilities include removing healthcare barriers through home visits, developing treatment plans, administering medication, scheduling appointments, coordinating care, and intervening in medical emergencies. The position also requires logging client medical care. Thresholds offers a collaborative and supportive culture with opportunities for training and professional development in areas like harm reduction and trauma-informed care. The schedule is primarily standard business hours with rare evening or weekend appointments.
Requirements
- Registered Nurse license
- Associate’s, Bachelor’s or Master’s degree from an accredited school of Nursing.
- Valid driver’s license
- Current car insurance
- Daily access to your own car
- Willingness to provide transportation to clients
- Passion for mental health advocacy
- Desire to provide compassionate care
- Effective communication and relationship-building skills
- Good writing skills
- Strong time management and organization skills
Responsibilities
- Provide medical and psychiatric support in the community.
- Develop treatment plans to meet client needs.
- Administer medication.
- Schedule appointments and coordinate care.
- Provide health education and training.
- Attend to medically related emergencies or crises, providing intervention as appropriate.
- Log client medical care.
- Conduct home visits to remove barriers to healthcare.
