About The Position

At Legacy Home Health (a Ventana Ventures company), we are honored to care for individuals who served our country through work with the Department of Energy and developed illnesses related to radiation, beryllium, or silica exposure. These patients qualify for care under the Department of Labor’s Energy Employees Occupational Illness Compensation Program Act (EEOICPA). Our mission is to provide compassionate, dependable, and personalized home health care that allows patients to remain safe and comfortable in their homes while receiving high-quality medical support. We are currently seeking a motivated and compassionate Outreach Specialist to join our growing team. This role is ideal for someone who enjoys building relationships, educating others about available healthcare services, and helping patients and families connect with the support they need. As an Outreach Specialist with Legacy Home Health, you will serve as an important liaison between our organization, patients, families, healthcare providers, and community partners. You will help expand awareness of our services while ensuring individuals eligible for care receive the information and guidance necessary to access support.

  • Basic computer and documentation skills required

  • Reliable transportation and valid driver’s license required
  • Must be able to read, write, and communicate effectively in English
  • Must pass a background check

Nice To Haves

  • Previous outreach, healthcare marketing, admissions, customer service, case management, or community relations experience preferred
  • Healthcare or home health experience preferred

Responsibilities

  • Building and maintaining relationships with referral sources, healthcare providers, community organizations, and patient advocates
  • Educating patients, families, and community partners about Legacy Home Health services and EEOICPA benefits
  • Conducting outreach efforts within the community to increase awareness of available healthcare services
  • Assisting prospective patients and families through the intake and referral process
  • Coordinating communication between patients, caregivers, office staff, and clinical teams
  • Following up with referrals and maintaining consistent communication with referral sources
  • Attending community events, meetings, health fairs, and networking opportunities as needed
  • Supporting patient recruitment and census growth initiatives
  • Maintaining accurate records of outreach activities, referrals, and communications
  • Assisting with marketing and community engagement efforts
  • Ensuring professionalism, confidentiality, and compassionate communication at all times
  • Collaborating with leadership and clinical staff to identify opportunities for growth and improved patient outreach
  • Responding promptly to inquiries from patients, families, and referral partners
  • Assisting with administrative tasks related to outreach and patient coordination
