The Outreach & Linkage Liaison is responsible for conducting outreach to identify eligible individuals and families for services, identifying alternative resources for those ineligible, and assessing eligibility for enrollment in programs/services. This role involves gathering necessary information, obtaining consent, facilitating document completion, and enrolling clients. The liaison collaborates with hospital staff, community members, and agencies to identify and outreach to potential clients, establishes and maintains internal and external relationships, and represents the agency at community meetings and events. A key focus is on violence prevention, providing immediate trauma-informed crisis intervention and emotional support to survivors of violent injury within the hospital setting, coordinating care with hospital staff and community partners, and facilitating continuity of support beyond discharge by connecting survivors to resources. The role also involves participating in program planning, public health emergency activities, and special projects.
